IMPLANT REMOVAL

Breast implant removal is conceptually a simple procedure, but there are some important considerations that effect the preoperative planning. The procedure can be performed through various incisions with or without a capsulectomy. Dr. Reid can help you decide which option is best for you and what pre-operative assessments or imaging is required. Any tissue removed during the capsulectomy is sent for pathologic examination.

ROLE OF FAT TRANSFER

For women who are concerned that removal and capsulecomy alone may result in a poor cosmetic outcome, fat transfer may be the ideal solution. Fat transfer at the time of implant removal and capsulectomy is a natural option for adding volume and enhancing breast shape. Fat transfer, also known as fat grafting, is a versatile procedure that Dr. Reid uses during facial cosmetic surgery, breast reconstruction and body contouring procedures.

During a fat transfer procedure, your own fat is moved from an area of relative excess to an area of deficiency, such as the breast. Liposuction is used to collect fat, and this portion of the procedure should be beneficial in and of itself. Fat is collected in a sterile closed system and then separated from other components of the lipoaspirate and at this point it is ready for transfer. Fat is injected in to the breast using specialized cannulas and a technique that maximizes contact between breast tissue and grafted fat and thus graft survival. This idea of spreading the grafted fat around within the recipient site tissue and avoiding the creation of ‘pools’ of grafted fat is fundamental to any fat grafting technique. With this in mind, and depending on the amount of breast tissue present, there will be technical limitations to the amount of fat that can be transferred during a single procedure. As a result, some women may choose to have a second fat transfer to the breast several months after the original surgery. The safety of fat transfer to the breast has been established, it does not increase a woman’s risk of breast cancer or interfere with breast screening.

During a consultation, we will help you to:

    • examine your breast tissue and determine the need for pre-operative imaging and other investigations
    • establish key considerations for the implant removal and capsulectomy
    • evaluate your breast tissue characteristics and the potential benefits of breast lift or breast fat transfer at the time of implant removal
    • set expectations for the final outcome and the potential benefit for a secondary procedure
    • select appropriate areas for liposuction and fat collection

All procedures take place at the Cambie Surgery Centre, a private surgical facility in Vancouver. These are day surgery procedures that take between 2 – 4 hours. Drains are often required following a capsulectomy. In addition, a post-surgical bra and a compression garment for the liposuctioned areas will be beneficial for 3 – 6 weeks. A typical recovery allows for return to work in 2 weeks and strenuous activities by 4 weeks. Final results are apparent by 3 months.
